MEMBER TERMS OF USE

By joining the Preferred Wellness Network, you will become a member of the network and agree to the following terms, conditions, and benefits set forth below:

• Individual membership price is $30 per year.  Family memberhship price is $40 per year.
• Individual members must be at least 18 years of age.
• Temporary membership card(s) can be printed from member profile page upon completion of registration.
• A permanent annual membership card will be mailed within 2 to 3 weeks of completed registration.
• Two (2) membership cards will be provided for family memberships.
• Additional membership cards may be purchased for $5.00.  For additional card(s), email request to memberships@preferredwellnessnetwork.com.
• Children between the ages of 14 - 18 will be listed on all family memberhship cards.
• All children listed must be under the age of 18 at the time of initial enrollment.
• Any member who wishes to cancel their membership may do so by sending an email to cancellations@preferredwellnessnetwork.com.
• Refunds for cancellation of memberships will be prorated.  A refund will be mailed for the unused term left on the annual membership.
• Members are entitled to any and all discounts offered by the business providers in the Preferred Wellness Network.
• All discounts and promotions will be advertised on the Preferred Wellness Network website.
• Members simply show their membership card to the provider to receive their discount.
• Provider may require member to show photo id to verify member identity.
• Provider may change discounts occasionally.

 

PROVIDER TERMS OF USE

By joining the Preferred Wellness Network as a business provider, you are entering into an agreement with Being Well, Inc.  The terms of the agreement are as follows:

• A webpage specifically dedicated to the provider within the main website.
• A link for the provider’s business website on preferredwellnessnetwork.com, which can be viewed by all visitors of the site.
• Provider will be required at all times to offer some form of discount or promotional item to the network’s members.  Provider’s promotion must either be solely available to the Network's members, or must be of better value than any other advertised promotions to the general public.
• Provider may offer as many promotional items or discounts, at any given time, as they choose.
• Provider will have a separate log-in page on preferredwellnessnetwork.com, where they can update their promotional items or discounts, update their bio, print flyers, track referrals, etc.

Being Well, Inc. offers an optional referral incentive program for all business providers.  Any business provider who chooses to participate in the referral incentive program agrees to the following terms and conditions: 

• A referral provider code will be assigned to each business provider.
• Provider can track their referrals in their personal profile page.
• Providers will receive 50% of referred member monthly dues, excluding any processing fees.
• Provider shall be paid commissions monthly for all active referred members from the prior month.  A check will be mailed to the provider the first week of each month with a detailed list of referred members.
• Provider is eligible to receive commissions as long as they are a provider in the network.  If at any time, for any reason, provider is no longer a part of the network; no further commissions will be paid by Being Well, Inc.
• Individual memberships are $30/year and family memberships are $40/year.
• Being Well, Inc. solely determines the membership dues.  Providers will be immediately notified of any membership pricing changes via provider email address on file.

*Being Well, Inc. offers a 100% satisfaction guarantee.  If you feel this service has not paid for itself either by new customers gained or by referral income generated, during your sponsorship term, we will give you an additional 1 year sponsorship absolutely free.  No questions asked!

** Provider may be terminated from network, without refund, at any time for breach of the abovementioned terms or for reasons deemed unfair or unethical by Being Well Inc.
